What is Intra-Cytoplasmic Sperm Injection (ICSI)?

Intra-Cytoplasmic Sperm Injection (ICSI) would be performed instead of regular In Vitro Fertilization (IVF) this method involves injecting a single sperm directly into an egg in order to fertilise it. With ICSI, the eggs are retrieved the same as if you were doing conventional IVF. However, the eggs and the sperm are then fertilized in the laboratory, by direct injection of a single sperm into each egg. The major development of ICSI means that as long as some sperm can be obtained (even in very low numbers), fertilisation is possible. The availability of this Intra-Cytoplasmic Sperm Injection, "ICSI" technique means that men whose sperm previously were too weak or too few to fertilize in vitro (IVF), now have no problem fertilizing their wife's eggs. The fertilization rate per egg using ICSI is about 70% despite the sperm being terrible, the fertilization rate per infertile couple is over 99% if the wife has adequate eggs, and the pregnancy rate per treatment cycle is over 50%. This is not significantly different from regular IVF with normal sperm. How Does ICSI Work?

Woman takes fertility drugs to stimulate her ovaries to produce more eggs, as for IVF. The eggs are then collected and each egg is injected with a single sperm from her husband.

If the husband can, he produce a fresh sperm sample on the same day as his partner’s eggs are collected. Otherwise sperm can be collected directly from the epididymis (a narrow tube inside the scrotum, where sperm are stored and matured) using a type of fine syringe. This is known as ‘percutaneous epididymal sperm aspiration’ or PESA or sperm can also be retrieved from the testicles, a process known as ‘testicular sperm aspiration’ or TESA. It is also possible to remove tiny quantities of testicular tissue from which sperm can be extracted. This procedure is called ‘testicular sperm extraction’ or TESE.
If man has a zero sperm count (other than caused by vasectomy), the chances of retrieving sperm surgically by PESA, TESA or TESE may be very low or at least uncertain. In this situation, if no sperm are retrieved; the options of having Donor insemination (DI) or In vitro fertilisation (IVF) with donor sperm can be considered instead.

In Vitro Embryo Hatching

The early embryo consists of a ball of cells surrounded by a protective outer shell called the zona pellucida. In order to implant into the lining of the uterus the embryo needs to break out of this shell by a process called “hatching”. Cryopreservation

Cryopreservation is a process where sperm, oocytes or embryos are preserved by cooling to low sub-zero temperatures, around −196 °C (the boiling point of liquid nitrogen). At these low temperatures, any biological activity, including the biochemical reactions that would lead to cell death, is effectively stopped.

The Hazrat e Maryam Fertility center provides sperm cryopreservation and storage (known as sperm banking) for men prior to chemotherapy, radiation, surgery, or other medical treatments that may jeopardize fertility. It is also available for men with poor sperm counts who may need a back-up sample stored and in cases where the man may be out-of-town at the time of egg retrieval. Sperm freezing is done by freezing several sperm samples in liquid nitrogen at sub-zero temperatures (typically -196°) where it can then be safely stored for several years. When the decision for conception is made, the frozen sperm can be thawed and used in combination with in vitro fertilization and can result in 40-60% pregnancy rate.

Egg freezing allows women to preserve eggs at a younger age for use when they are older. Whether eggs are being cryopreserved as a medical emergency in the setting of a newly-diagnosed cancer or for personal reasons, the technology holds great promise for women of reproductive age. During an egg-freezing cycle, a patient will go through many of the same steps that are involved in a typical IVF cycle: ovulation stimulation, ultrasound monitoring and egg retrieval. After egg retrieval, the eggs will be cultured for a few hours and then frozen the same day for future use.

Untile a sufficient number of births is reached and adequate outcome data are collected, oocyte cyropreservation should continue to be considered experimental and should be performed under the oversight of an institutional review board.

Embryos are eggs that have already been fertilized with sperm. GIFT ZIFT

Gamete Intra-Fallopian Transfer (GIFT) in shiraz iran

This technique is used in cases in which at least one fallopian tube is normal. The eggs and the sperm are deposited in the fallopian tube, and therefore, the normal physiological fertilization process is mimicked. The whole procedure is performed under general anaesthetics. The preparation is similar to In Vitro Fertilization, except for egg collection being performed by laparoscopy.

Gender Selection

While having a healthy baby is desirable for any family, "Gender Selection" has also attracted great interest and controversy over the years. A few of the reasons why couples may wish to request a specific gender, is the desire to balance the gender among members of a family; to avoid genetic disorders that are more common in one sex (sex linked disorders); or sometimes, to accommodate cultural or social norms. These factors may encourage families to choose traditional or new treatments for gender selection. In the absence of a boy or girl in the family, many couples face multiple cultural issues and may try to have the gender that they wished for all their lives. At Hazrat-e Maryam Fertility Center, we support sex selection programs  in order to help couples to balance the gender of the family for their second child and beyond. There are different types of procedures used for sex selection, most of which involve separating X and Y chromosomes from sperm or involve the selection of embryos with male or female DNA in order to produce a baby of the desired gender.

Preimplantation Genetic Diagnosis (PGD) Method in shiraz iran

The method of choice for sex selection in our center is preimplantation genetic diagnosis (PGD), using FISH or PCR techniques. This is a reliable and accurate method for selecting the desirable gender with 95% success rate. As with all ICSI procedures, eggs are removed after ovarian stimulation and fertilized in a laboratory using microinjection method. To make use of PGD the couple has to go through routine IVF procedure so that several embryos can be tested when they are just three days old. After fertilization, the embryo will  grow to eight cells. Typically, one to two cells are removed by biopsy and are tested by FISH or PCR techniques, determining embryo's gender. Finally the embryos with the desirable gender by the couple will be transferred into a woman's uterus.

Ericsson Albumin Method in shiraz iran

Preimplantation Genetic Diagnosis (PGD)

Preimplantation genetic diagnosis was developed to reduce the risk of passing on a genetic disorder in high-risk couples. The aim was to enable them to start a pregnancy knowing that it was unaffected by a disorder. In the PGD procedure, going through routine IVF, several embryos are tested before being implanted in the womb, when they are three days old. The process includes ovary stimulation through medication, egg retrieval and fertilisation in the laboratory. Over the following three days, when the embryos reach 6 to 8 cells, it is safe to remove one or two cells for diagnostic testing, which takes 24 to 48 hours. Usually, embryos which pass the test are transferred to the mother five days after egg collection.

There are two main techniques used for diagnosis. In the case of single gene disorders like myotonic dystrophy or cystic fibrosis, DNA is extracted from the individual cells and analysed after amplification (increase in the number of copies) by the Polymerase Chain Reaction (PCR). For chromosomal disorders such as translocations or for sex-linked disorders such as hemophilia, fragile X- syndrome and most neuromuscular dystrophies, Fluorescent In Situ Hybridisation ( FISH) is used.

Once the PGD procedure is completed and embryos free of genetic problems have been identified, implantation will be attempted through embryo transfer, intracytoplasmic sperm injection (ICSI) or zygote intrafallopian transfer (ZIFT). While PGD helps reduce the chance of conceiving a child with a genetic factor, it can not completely eliminate this risk. Surgical Male Fertility Treatments
